Philippine Passport
A. Passport Renewal Requirements:
- Personal Appearance
- Duly accomplished passport application form (Adult or Minor form)
- Bring and present the recent passport and one (1) photocopy of the port data page
- Processing Fee of QR240 (cash only)

Additional Requirements for Minor Applications (less than 18 years of age)
- Copy of Report of Birth issued by the Embassy (if born in Qatar) or copy of Philippine Statistics Authority (PSA) [formerly National Statistics Authority – NSO] issued birth certificate (if born in the Philippines);
- Copy of Qatar ID (QID) or passport of the accompanying mother. If accompanied by the father, copy of parent’s marriage certificate from the PSA/NSO as additional requirement to establish paternity.
B. Passport Requirements for Newborn
Follow requirements for Report of Birth (see under Consular Services-Civil Registry-Birth)
Note: Passport processing time is 30-45 days (average). Additional requirements may be requested by the Embassy in compliance with relevant/prevailing rules and regulations as prescribed by the competent authority.
C. Additional Requirements:
1. Replacement of Lost Philippine Passports:
- Present original and one (1) photocopy of birth certificate from the Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA;
- For married women, original and photocopy of the iage certificate from Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA;
- Police report (with English translation);
- Photocopy of any Philippine-issued ID, such as driver’s license, voter’s ID, Professional Regulatory Commission (PRC) ID, school/college ID, etc.;
- Photocopy of data page of lost passport, if available;
- Duly accomplished passport application form (Adult or Minor form);
- Affidavit of Loss;
- Personal Appearance;
- Payment (see below).
Fees for Lost Passport (cash only – please bring exact amount):
- QR100 – for Affidavit of Loss;
- QR100 – for verification, if passport was not issued at Philippine Embassy in Doha;
- QR600 – for replacement.
Please take note that there will be a 15-day clearing period prior to the processing of application for replacement of lost valid Philippine passport.
2. Renewal of Green/MRRP or Maroon/Machine Readable Passports:
- Present original and one (1) photocopy of birth certificate from the Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA;
- For married women, original and photocopy of marriage certificate from Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA.
3. Change of Surname (For married women):
- Present original and one (1) photocopy of DFA-authenticated marriage certificate; or
- Present original and one (1) photocopy of Report of Marriage issued by the Embassy (if less than 1 year from date of marriage);
- If keeping (opt to use) maiden name, same two requirements as above (3.a/3.b).
Note: If applicant is changing information on Qatar residence ID (QID), please refer to requirements on Certification (see Authentication and Notarials page)
4. Reversion to maiden name (For married women):
- Present original and one (1) photocopy of DFA-authenticated birth certificate;
- If widowed, original and photocopy of duly-authenticated Death Certificate of the deceased husband;
- If marriage is annulled, one (1) original and one (1) photocopy of DFA-authenticated marriage certificate, and one (1) original and one (1) photocopy of DFA-authenticated certified true copy of Certificate of Finality and Court Order;
- If divorced, certified true copy (CTC) of the Divorce Decree duly-authenticated by the Philippine Embassy or Consulate where the divorce was obtained or by the concerned foreign diplomatic or consular mission in the Philippines, or duly-authenticated or certified true copy of Philippine court recognition of foreign divorce decree.
Note: If applicant is changing information on Qatar residence ID (QID), please refer to requirements on Certification (see Authentication and Notarials page)
5. For converts who would like to use their Muslim name:
- Present original and one (1) photocopy or duly authenticated Birth certificate bearing the Muslim name;
- Present original and one (1) photocopy of duly authenticated National Commission on Muslim Filipinos (NCMF) or Office on Muslim Affairs (OMA) Certificate of Conversion;
6. If applicant has reacquired his or her Philippine citizenship:
- Present original and one (1) photocopy each of Dual Citizenship – i) Identification Certificate, ii) Order of Approval and iii) Oath of Allegiance issued by the Philippine Embassy or Consulate with jurisdiction or by the Bureau of Immigration (BI)
